STEP 1: Call us @ 1-709-279-2554 Provide us your name and Phone #, and let us know you're looking for accommodations.
STEP 2: APPLY FOR A HEALTH ASSESSMENT & FINANCIAL SUBSIDY by calling Community Intake: 1-833-210-8491. They will ask you preliminary questions to see if you are a candidate for a financial subsidy.
STEP 3: Schedule an appointment with a Public Health Care Nurse or Social Worker for an assessment. This assessment will identify your needs and assess you as either a Level 1, 2, or Enhanced Level Care.
STEP 4: You’ll receive an application from Eastern Health to complete and submit, which determines the amount of financial subsidy you’re eligible to receive.
STEP 5: You’ll receive a letter indicating whether you have been approved for a subsidy. If you are not approved for a subsidy, we'll work directly with you on next steps, call us at the # above.
